Print your adopted animal's health record

Medical records gives you a printable health record for every animal you adopted from the shelter: its details, allergies and ongoing conditions, and its full vaccination history while it was with them. Take it to your own vet.

Print or save the record

1. Select Medical records in the portal menu.
2. Select Print / Save as PDF. Choose your printer, or save as PDF from your browser's print window.

The printout leaves out the portal menu and buttons.

What's on it

For each animal:

• Species, breed, sex, colour, whether it's spayed or neutered, and its date of birth.
• Allergies & ongoing conditions, or None recorded.
• Vaccination history: each vaccine with the Date given, Next due, Lot, Manufacturer, Site and Given by.

It covers what the shelter did while the animal was with them. It's not a complete lifetime chart, and it doesn't include fees or payments (those are under Fees & payments).

If something looks wrong or incomplete, call the shelter.
