On this page

Medical records gives you one printable health record for all your horses: identification, allergies and ongoing conditions, the current Coggins and health certificate, vaccinations, and farm call notes. Use it when a show office, trainer or new yard asks for your horse's records.

Open and print the record

  1. Select Medical records in the portal menu. The Equine health record opens, prepared in your name and dated today.
  2. Select Print / Save as PDF. Choose your printer, or choose to save as a PDF in your browser's print window.

The printout keeps your practice's name, address and phone number at the top, so whoever reads it knows where it came from. The portal menu and buttons are left off the paper.

What's in it

For each horse:

  • Breed, sex, colour, height, use, date foaled and where it's kept, plus registration, brand or tattoo, and Markings if recorded.
  • Allergies & ongoing conditions.
  • Travel paperwork — current standing: the latest EIA (Coggins) test and health certificate, with Valid to or EXPIRED and the date. This is a summary, not the certificate itself.
  • Vaccination history: each vaccine with the date given, next due date, lot, manufacturer, site and the vet who gave it.
  • Farm call history: each visit's date, place, vet and notes.

A horse that has died is marked No longer with us and stays on the record.

Your practice chooses which of these sections appear. If one is missing, it isn't shared online.

If your practice provides records by phone rather than online, the page says so instead.
